Community Service Award Nomination Form

DEADLINE for Applications is April 4th, 2025.
Individuals and organizations involved with seniors may submit nominations to the Agency.

Community Service Award
Each year, an organization within Ashland, Crawford, Huron, Knox, Marion, Morrow, Richland, Seneca, or Wyandot is selected and recognized for their outstanding contributions and positive image they represent of aging. Nominators will receive a confirmation of receipt via email. 

Nominators will receive confirmation of receipt by email. If you do not receive a confirmation, please contact Sue Montgomery at 567-247-6458.

If your nominee is selected, the Award Ceremony will be held at the Ohio District 5 Area Agency on Aging (2131 Park Avenue West, Ontario) on August 15th, 2025, at 10:00 AM.

You will receive your invitation via email. *Please note that you will only be notified if your nominee is selected.*
